Track 2 "Mini-Fellowship" - Session #2

Hospitals participating in this mini-fellowship have identified one or more key patient and family engagement strategies that they want to implement or bolster within their organizations.  Monthly webinars through March 2019 will allow hospitals to share progress with their peers in implementing their actions plans and to help stay on track.

About this Collaborative:

KHC is extending its fourth breakthrough collaborative with national expertise, a roadmap and individualized support designed to help organizations learn and implement key Patient and Family Engagement (PFE) concepts and practices, and to take steps to establish an active Patient and Family Advisory (PFA) Program, including a Patient and Family Advisory Council (PFAC) and patient/family representatives on quality committees and projects.  All hospitals in the KHC Hospital Improvement Innovation Network are eligible to participate.

About this Sprint:

Kansas hospitals that are ready to discover opportunities and test methods to help prevent falls with injury are meeting monthly during the Kansas Healthcare Collaborative’s HIIN Falls Prevention Sprint. This six-month sprint, conducted with Jackie Conrad, BSN, MBA, improvement advisor, Cynosure Health, will conclude March 21, 2019.

Objectives for this six-month sprint are to assist participating hospitals in:

  • Identifying opportunities for PDSA improvement cycles through use of the HRET HIIN Falls Process Discovery Tool.
  • Applying targeted strategies for involving patients and families in fall prevention, such as validating patient/family understanding and adopting or improving the post-fall huddle process.
  • Engaging in the HRET HIIN Up Campaign by designing "Get Up" strategies for engaging front line staff in preventing injuries from falls and immobility.
