This month's session is for improvement leaders who are looking for an effective solution for improvement teams to implement change quickly.  Stephanie Sobczak, MS, MBA, project manager of UW Health, Madison, Wisc., will share a structured approach which marries elements of project management along with established improvement methods.  Stephanie will share meeting agenda templates designed to document standard work and establish clear, standardized expectations for approaching patient safety projects or other opportunities for improvement.  As part of this presentation, those attending will receive a toolkit easily adapted to a local setting.  The method is well suited for experienced quality leaders and those newer to improvement.

About our presenter

Stephanie Sobczak, MS, MBA, has worked in health care quality improvement since hospitals were implementing "Total Quality Management."  Her first career was in the clinical setting as an athletic trainer and exercise physiologist in a rural hospital.  She obtained an MBA with a focus on quality improvement, then moved on from her clinical role to focus on improvement full time.  She has worked for an HMO, the Wisconsin Hospital Association, and now University of Wisconsin Health Health System in Madison.  An engaging public speaker, she has launched IHI's Transforming Care at the Bedside programs for six state Hospital Improvement Innovation Networks (HIINs), has been a Forward Award Baldrige examiner, and is a certified Just CultureTM Champion.  Her passion is patient safety and culture change.

This webinar—hosted by the Kansas Healthcare Collaborative in partnership with the Center for Practical Bioethics, the Kansas Foundation for Medical Care, and the Health Quality Innovation Network—will focus on Advance Care Planning.

Advance Care Planning 101

This session for physicians, mid-level practitioners, nurses, social workers, and other health care professionals will cover the purpose and process of advance care planning with an emphasis on normalizing the process for all adults and encouraging attendees to complete their own advance care planning documents.

Presenters from the Center for Practical Bioethics will provide an overview of the living will, the appointment of the agent, the duties of both the declarant and agent that appear in these documents, and the relevant laws in both Kansas and Missouri. The session will conclude with a question and answer period.


  • Differentiate process and purpose, types of products
  • Identify the duties of the declarant and the agent
  • Describe how the ACP process is reflected in the law in Kansas and Missouri
