Kansas hospitals participating in the KHC Hospital Improvement Innovation Network (HIIN) are encouraged to attend our monthly virtual meetings, usually held the fourth Wednesday of each month from 10 to 11 a.m. CT. This month's event will take place Wednesday, August 28, 2019.
Our guest speaker will be Kim Werkmeister, BA, RN, CPHQ, CPPS, improvement advisor, Cynosure Health. Kim is a nurse leader and national expert in the reliable implementation of patient safety and process improvement strategies.
During this session, Kim will review Key Strategies for Reducing Avoidable Readmissions, including using data to inform improvement activities, improving transitions of care, delivering enhanced services based on need, and collaborating across the continuum. She also will connect us with many resources available to guide improvement in readmissions reductions.
Monthly KHC HIIN meetings provide direct access to subject matter experts, peer-to-peer sharing and KHC HIIN project leaders. This also is a good opportunity to stay in the know about the project, progress to date, resources and opportunities in quality and patient safety.
